Waiver and Release of Liability
Last updated: October 29, 2026
This is a binding legal agreement for One Stop Goaltending programs, including camps, clinics, semi-private and private sessions, training and videoing, and for all activities and events related to on-ice and off-ice goalie training held at various arenas and facilities, including, but not limited to, the ice surfaces, changing rooms, exercise rooms, parking lots and all walkways at the arenas holding One Stop Goaltending events. As a Participant in any of the on-ice programs, on-ice preparation and pre- and post-ice activities, the Participant (and, if the Participant is under 19, their parent or legal guardian) acknowledges and agrees to the following terms.
Disclaimer
"The Organizer", One Stop Goaltending Inc., Michael Hails, other instructors, volunteers, Participants, agents, sponsors, owners/operators of the facility, and any other representatives (of "The Organizer") are not responsible for any injury, damage or loss of any kind suffered by a Participant during, or as a result of, any practice activity or related event, caused in any manner whatsoever including, but not limited to, the negligence of "The Organizer" and its participants.
The Organizer reserves the right to not allow any person to register or participate, in whole or in part, where this agreement has not been accepted by the Participant or, if the Participant is under 19, by their parent or legal guardian.
Description of Risks
In consideration of my participation in such programs, activities and events, I hereby acknowledge that I am aware of the risks and hazards associated with or related to ice hockey and associated sports training. The risks and hazards of ice hockey include, but are not limited to, injuries from:
- Exerting and stretching various body muscle groups;
- Vigorous physical exertion, rapid movements and quick turns and stops on the ice;
- Strenuous cardiovascular workouts;
- Collisions with the hockey boards, goalie nets and ice;
- Being struck by hockey sticks and pucks;
- Physical contact with other participants, resulting in injuries to the eyes, teeth, face, head and other parts of the body, bruises, sprains, cuts, scrapes, breaks, dislocations and spinal cord injuries which may render me permanently paralyzed;
- Variations in the ice surface; and
- Using various training equipment, weights, skipping ropes, stretch ropes and benches.
Furthermore, I am aware:
- That injuries sustained in ice hockey can be severe;
- That hockey is a contact game. It is understood and accepted that body contact will occur and that some body contact can be either intentional or purely accidental. In either case, I am fully responsible for my own physical safety in this competitive environment and I assume all risks associated with my participation;
- That I may experience anxiety while challenging myself during the activities;
- That I can lessen the chance of severe injury by wearing certified, hockey-specific protective equipment, including but not limited to shin pads, elbow pads, shoulder pads, helmet, hockey gloves, hockey pants, athletic supporter and neck guard, and/or gear commonly associated with the goalie position;
- That despite the rules in place, violations and/or accidents will occur and may result in injury; and
- That my risk of injury increases as I become fatigued or distracted.
Face Protection
Goaltenders: I understand that as a goaltender I must wear, and keep in the standard playing position at all times, a CSA-approved goalie mask/helmet commonly used and accepted at my level of play (including but not limited to full bars or cat-eye face masks).
Skaters and shooters: It is at the discretion of each skater or shooter to wear a junior-style visor (partial face cover) instead of a cage (full face cover). Anyone who chooses not to wear a full cage is exposed to a much greater risk of injury, and acknowledges that they have read and understood the conditions described in this agreement in making that choice.
Shooters
Participants may be asked to bring their own shooter. By registering, the parent or legal guardian of the registered goalie accepts responsibility for any shooter they bring, and confirms that the shooter (and, if the shooter is under 19, the shooter's parent or legal guardian) is aware of the risks and terms in this agreement.
Release of Liability
In consideration of "The Organizer" allowing me to participate, I agree:
- To assume all risks arising out of, associated with, or related to my participation;
- To be solely responsible for any injury, loss or damage that I might sustain while participating; and
- To release "The Organizer" from liability for any and all claims, demands, actions and costs including, but not limited to, breach of contract, breach of statutory duty of care or breach of any other duty, that might arise out of my participating, even though such risks, injuries, loss, damage, claims, demands, actions or costs may have been caused by the negligence of "The Organizer".
Permission to Use Photos and Video
All sessions may be filmed. I / We grant One Stop Goaltending permission to record and use photographs and video of the Participant for coaching and training purposes, including sharing footage with the Participant through CoachNow. Use of photographs or video on our website, social media, or in advertising follows the choice made at registration ("Video on social media: Yes / No").
Where permission for website or social media use has been given, the grant includes the right to edit the images at the discretion of One Stop Goaltending. I / We understand that such materials could circulate worldwide, that there will be no compensation for this use, and that I / We will not be given the opportunity to inspect or approve the finished material. In granting this permission, I / We release One Stop Goaltending from any liability that may arise from the permitted use of the images.
General Release
In consideration of the Participant and their parent/guardian being permitted to register the Participant and participate in any One Stop Goaltending program (camps, clinics, semi-private and private sessions), we hereby forever release and discharge One Stop Goaltending Inc. and its directors, agents, employees and any person or corporation connected herewith from all manner of actions, injuries, damages, costs, claims or demands which we shall or may hereafter have, suffer or receive by reason of such participation. This release shall be binding on our heirs, assigns, executors and administrators. It is agreed that One Stop Goaltending Inc. does not and shall not be considered to guarantee or warrant any equipment used in conducting the program. It is further agreed that One Stop Goaltending Inc. is not responsible for lost hockey equipment. There are no exceptions.
Cancellations and Refunds
No refunds will be provided within 48 hours before the start of the session or program unless a medical certificate accompanies the request. All refunds are subject to a $50.00 service charge.
Accepting this agreement confirms that you have read and understand all of the above, including our cancellation policy.
